Position Title: Geriatric Partial Care Counselor (All day Wednesday only)
Responsible To: Director of Partial Care
Qualifications:
Bachelor's degree. Minimum of one year experience working with geriatric population. Must have appropriate professional license and/or certification, good communication and managerial skills, be able to organize own time well, work independently or with a team. Driving record must be accepted by NewBridge Services' auto insurance underwriter. Some supervisory experience is helpful.
Responsibilities:
- Assist the Director of Partial Care with the overall development, coordination and implementation of geriatric program.
- Conduct group activities designed to meet the needs of the geriatric clients who are housed in group/boarding/private homes in our area.
- Serve as member of multi-disciplinary team for the purpose of assessing client strengths and needs, and aid in the development and implementation of each client's individual treatment plan.
- Collect data and maintain documentation as related to direct service and program functioning.
- Provide individual and group counseling and support required for crisis management and case management services.
- Supervise van driver when utilized.
- Attend supervision, treatment team, in-service and other staff meeting, and participate in agency committee work as assigned.
- Assist the Director in maintaining and requisitioning needed material and supplies.
- Work with the Director of Partial Care in evaluating, modifying and further developing the program to meet the needs of the clients.
- Transport clients.
- Other duties as assigned.
