Position Title: Case Manager (ICMS)
Responsible To: ICMS Team Leader
Qualifications:
A bachelor's degree in social work, psychology, counseling, nursing, or related field, plus 2 years post degree work experience in the provision of mental health or related services to persons with mental illness. Team orientation and creative problem solving ability required. CAC preferred. Degree requirements may be waived for Peer Associates.
Responsibilities:
- Provide consumers with direct assistance including assessment, service planning, linkage with requisite services, monitoring of service delivery, psychoeducation and support, family and significant other education and support, crisis intervention assistance designed to divert hospitalization, systems advocacy, direct assistance in obtaining the basic necessities of living, direct assistance with structuring and performing basic daily living activities, symptom assessment, management and supportive counseling, and substance abuse education and intervention.
- Actively participate in GPPH discharge planning process, including face-to-face contact with the consumer within five days of admission, completion of a community assessment, participation in the consumer's 72-hour or comprehensive treatment plan meeting, charting as significant events occur, and participation in discharge planning.
- Accurately document service provisions on a timely basis.
- Conduct planned service activities on evenings and weekends according to schedules developed by the Team Leader.
- Supervise the daily activities of Peer Outreach Specialists.
- Participate in supervision, team meetings and on assigned NewBridge committees.
- Maintains licensure and professional development through attendance at seminars and workshops related to the mental health field.
