Position Title: Outreach Case Manager, NewBridge SAIL (Senior Assistance for Independent Living)
Responsible To: Team LeaderQualifications:
- Bachelor's degree in Social Work, Psychology, or related field (or) high school diploma with two years of experience in programs for seniors or community outreach/case management activities.
- NJ Certified Social Worker (CSW) preferred
- Valid NJ drivers license
- Good communication skills
Responsibilities:
- Assess client needs and provide supportive counseling, advocacy, and follow-up to ensure successful linkage with other services/agencies.
- Transport clients to medical and social service appointments within the community as needed, using NewBridge Services vehicles.
- Provide outreach services through home visits, contact with significant others and other community resources to develop, increase and maintain client participation in services.
- Liaise with other service providers for the purpose of promoting continuity of care within the least restrictive environment.
- Collect data and maintain documentation per all client-related activities.
- Maintain a current file on community resources for client services.
- Attend weekly staff and supervision meetings, and participate in internal committee meeting as assigned.
