Building Essential Life Skills for Lasting Independence
Community support services focus on developing the practical skills needed for successful independent living while managing mental health symptoms. Our comprehensive approach addresses three essential areas of support:
- Wellness and Recovery Support: This included medication management assistance, symptom and illness management, relapse prevention, co-occurring disorders treatment, crisis intervention with an on-call team and connection to legal assistance.
- Daily Living Skills Training: This covers daily living and healthy lifestyle practices, communication and goal-setting, financial management, leisure time management, landlord and neighbor relationships and using public transportation.
- Community Integration Services: These services include support in clients' homes or community settings, coordinating care with medical and psychiatric providers, assisting with benefits and entitlements, and preparing individuals for employment or volunteer opportunities.
Through this personalized skills-based approach, clients gain confidence in navigating daily challenges while building the foundation for long-term independence. Many clients come to us through referrals from hospitals, health care professionals or our own mental health evaluations. We welcome individuals at various stages of their mental health journey, offering personalized support that reflects their needs and priorities.

Our Community Support Process
Community support services begin with an evaluation and continue with individualized support. The goal is to match services to each person’s needs as they work toward greater independence:
- Initial Assessment and Enrollment: The process begins with an evaluation to confirm eligibility, which requires a diagnosis of severe and persistent mental illness from a qualified practitioner. Referrals usually come from hospitals, health care professionals or NewBridge mental health evaluations.
- Personalized Care Planning: Our team works with you to set goals and create a recovery plan that supports independence and community life.
- Ongoing Support and Coordination: Services are provided in your home or community, with close coordination alongside your medical and psychiatric providers.
Our team remains available for crisis intervention and on-call support when needed, making sure resources are in place during difficult times. This coordinated approach helps residents maintain stability and work toward greater independence and connection in the community.
