With decades of experience in housing supports, NewBridge was one of 43 nonprofits selected throughout the state to provide this critical service. NewBridge delivers the program in Bergen, Morris, Passaic, Sussex and Warren counties.

Who Our Housing Support Program Serves

The NJ FamilyCare Housing Supports Program serves people enrolled in Medicaid who face homelessness and have a health challenge such as a chronic condition, mental illness, substance use, or pregnancy. To qualify, individuals must be enrolled in one of five Medicaid Managed Care Organizations: Aetna Better Health of New Jersey, Fidelis Care, Horizon NJ Health, UnitedHealthcare Community Plan, or Wellpoint. 

Our team provides specialized supports needed to address these challenges, with an approach that recognizes housing stability as essential to accessing and maintaining other community services.

Our housing support program follows a carefully structured process designed to address both immediate housing needs and long-term stability goals. Each step is personalized to your situation and delivered by our experienced team:

  • Housing Search Guidance: We work with you to identify suitable housing options that meet your needs, preferences and budget while considering accessibility and proximity to essential services.
  • Application Support: Our team assists with completing housing applications, gathering required documentation, and communicating with landlords or housing authorities.
  • Move-In Preparation: We help coordinate move-in logistics, connect you with necessary resources and ensure you're prepared for this important transition.
  • Ongoing Case Management: We provide continued support to help you maintain housing stability.

This approach ensures that housing is the first step in an ongoing journey toward stability and wellness. Through the 2025 initiative and our collaboration with state departments, we provide vital support as part of New Jersey’s broader effort to address homelessness and improve health outcomes.
