Housing Support Program in New Jersey
Finding Stability Through Safe Housing Programs
Homelessness or housing instability is a crisis — especially when compounded by an individual's medical or mental health concerns. NewBridge Services understands that health and housing are deeply connected. NewBridge addresses this as a provider of the NJ FamilyCare Housing Supports Program, helping Medicaid-enrolled individuals search for, secure and sustain safe housing.
With decades of experience in housing supports, NewBridge was one of 43 nonprofits selected throughout the state to provide this critical service. NewBridge delivers the program in Bergen, Morris, Passaic, Sussex and Warren counties.
Our NJ FamilyCare Housing Supports Program ensures you receive the support you need at every step:
- Guidance in housing searches tailored to individual needs
- Application assistance to navigate complex housing processes
- Support with move-in preparation

Who Our Housing Support Program Serves
The NJ FamilyCare Housing Supports Program serves people enrolled in Medicaid who face homelessness and have a health challenge such as a chronic condition, mental illness, substance use, or pregnancy. To qualify, individuals must be enrolled in one of five Medicaid Managed Care Organizations: Aetna Better Health of New Jersey, Fidelis Care, Horizon NJ Health, UnitedHealthcare Community Plan, or Wellpoint.
Our team provides specialized supports needed to address these challenges, with an approach that recognizes housing stability as essential to accessing and maintaining other community services.
